Bill Gates used to instruct his Microsoft Research group that if they weren&#039;t failing at least 50% of the time, they weren&#039;t pushing the boundries far enough.   The best work on the subject is &quot;To Engineer is Human&quot;:  The Role of Failure in Successful Design&quot; by Henry Petrovski.</description>
